Letter from Pets in Need staff alleges major errors led to puppy deaths

The deaths of seven puppies while they were being transported by the nonprofit on Aug. 2 were caused by multiple senseless violations of law and protocol, according to a letter to the organization's board.

If the allegations stated in this article prove to be true then disciplinary action needs to follow for those who were involved from the top of the org chart to its bottom, such discipline that could even result in termination of one's job. No doubt there will be a lot of attempts to shift blame and point fingers, but in my experience fiascos like this don't just happen without there having been similar system failures before this event. If you simply punish those who screwed up and not look at the system that allowed this to happen, nothing has been learned.
